Basic Education Enrollment Form and Confirmation Slip
Pursuant to DepEd Memorandum 32, s. 2024, incoming Kindergarten, Grades 1, 7, and 11 enrollees, as well as transferees across all grade levels, will need to complete the Basic Education Enrollment Form. On the other hand, learners who are continuing their studies from Grades 2 to 6, Grades 8 to 10, and Grade 12 will simply fill out a Confirmation Slip to confirm their continued enrollment. Lastly, for individuals enrolling in the Alternative Learning System (ALS), a dedicated Modified Alternative Learning System (ALS) Enrollment Form is required.
